Understanding Stiffness, Swelling & Setbacks – What’s Normal After a Knee Replacement?

It’s a completely valid question—especially when the healing process doesn’t feel as smooth as expected. Let’s break down the three big “S”s of recovery: stiffness, swelling, and setbacks—what’s part of the normal healing journey, and what might need attention.

Stiffness: Common, But Treatable

Some level of stiffness is very common after a knee replacement. It’s your body’s natural response to surgery, inflammation, and healing. In fact, most people feel tightness or reduced flexibility at some point.

In the first few weeks, it’s normal to experience:

  • Difficulty fully bending or straightening the knee
  • Tension when standing or walking
  • Tightness after sitting or lying down

What’s normal: Stiffness that improves gradually with regular movement, stretching, and physiotherapy guidance.

What’s not normal: Stiffness that gets worse over time, or severely limits your ability to walk, stand, or carry out daily activities.

The good news? Early and consistent physiotherapy can make a significant difference in preventing long-term stiffness and improving your comfort and mobility.

Swelling: A Natural Part of Healing

Swelling is another normal part of the healing process after a knee replacement. You may notice:

  • Puffiness around the knee joint
  • Swelling in the lower leg, ankle, or even the foot
  • A warm, tight, or “full” feeling in the knee

Tips to manage swelling effectively:

  • Elevate the leg above heart level for 20–30 minutes a few times a day
  • Apply ice packs (with a cloth barrier) for 15–20 minutes at a time
  • Move regularly, but avoid long periods on your feet
  • Use compression stockings if advised by your healthcare provider

What’s normal: Gradual reduction in swelling over several weeks, with occasional flare-ups.

What’s not normal: Sudden or severe swelling, redness, warmth, or pain in the calf—these could be signs of a blood clot and need urgent medical attention.

Setbacks: Normal—But Manageable

Recovery isn’t a straight line. Most people experience ups and downs—one day feels great, the next less so.

Setbacks can happen due to:

  • Overdoing it—too many stairs, too much walking, or lifting
  • Not enough rest, ice, or elevation
  • Weather changes (yes, your new knee may “predict” the weather!)

What’s normal: Temporary pain, swelling, or fatigue after increased activity.

What’s not normal: Ongoing decline, signs of infection, or pain that doesn’t improve with rest or medication.

The key is to listen to your body, pace yourself, and stay consistent with your exercises.
